ALV Grid Display for Header (TOP-OF_PAGE)

Hi,

I am using ALV_GRID_DISPLAY to print the report output.

Now I want to print the top-of-page with the header,as follows

Company code name: Customer Statement of Account

From: To:

City: Postal code:

like the horizontal one as we see in basic list.

Could anyone help me to print the output as above in the grid display.
