Issue with ORDCHG idoc for PO

I found an interesting issue in the ORDCHG idoc. There are 2 parts to it.

1. I did not configure EKES-EINDT as part of change relevant for ORDCHG triggering, but output for ORDCHG was triggered successfully even when only the confirmation date is changed.

2. When this ORDCHG idoc is triggered, the idoc item contains a similar item with action '001' being triggered previously. In another words, ORDCHG was triggered twice with action code '001' for the same item.

Any idea what happens? I can't find any related OSS notes on this. Something wrong with how SAP 'remembers' change documents in PO?
