User Exit MM06E005 Screen Error

hi

i activated user exit MM06E005 with refer to SAP Note -407975,

in it i also included n activated two screen SAPLXM06 0101

and SAPLXM06 0111,

but i have not changed INCLUDE ZXM06TOP, because i am not able to find this include ,

now i activated this project.

but now when i am going in T.Code ME21N/ME22N Customer TAB it is giving ERROR message "SAPLXM06 0101 must be an include screen (screen error)"

plz help its urgent.
