Namespaces for new objects for the countries without IS-H localization

Need the explanation for the following item:

There is neither central SAP localization of Healthcare for Russia nor the local one. So in this situation, SAP presentation material "Technical procedure for IS-H country localization V4B" on the page 9 suggests us that we should take the namespace "ISH_RU_HEALTH1". It is not clear for me how many symbols of this namespace must be used in our own programs names? For example: ISH_RU_HEALTH1_PROGRAMNAME or ISH_RU_HEALTH_PROGRAMNAME, or it is possible to name ISH_RU_PROGRAMNAME?
