Just follow the upgrade guide.

Download the revelent guide from the below mentioned link depending upon your database and SAP version.

http://service.sap.com/instguides

Guide contains all the required information including your hardware, software requirements, required information about the patch level, etc and phase wise informtion about the upgrade.

There are a list of OSS notes mentioned in the guide. Please go through them carefully to get additional information.

All your modified data can also be transfered from 4.6C to ECC 6.0. Please get back incase if you need any specific information at any point of your upgrade.
