Use Crystal Reports to fill an existing PDF form from SQL Database.

I have several different insurance forms in pdf format that I would like to fill from our SQL database. Is this possible in CR?

The image approach would not get you a true PDF form (the user would not be able to use it as a form).

At least one of the 3rd-party Crystal Reports desktop schedulers listed here allows you to use Crystal Reports to
a) fill existing PDF forms with data from the report or
b) create pdf forms on the fly (with sections based on the report grouping and detail structure and default values based on report data)

Assuming you do not want to recreate the PDF in Crystal but simply add data to an existing document.

Create an image of the PDF using a snipping tool.

Create a new blank report connected to data source as required.

Insert image of PDF in report header and in section expert check underlay following sections.

You can then see layout of PDF and place fields in relevant positions. It will be an iterative process as sizing the PDF image can take a few attempts to get it right.

Sorry Ido's comment prompted me to expand.

I should have added that once you have fields in correct location. You load printer with your preprinted PDFs and then print report onto the PDF. The relevant boxes or locations on PDF will now have the data.
