Problem with the adobe form created

Hi ,

I have created an adobe form which I would be sending to users inbox..(Time being It is my inbox)..Then there are some questions with four check boxes against it..I need to fill this check box and save it .

when I fill this check boxes it takes the values..but when I save it it says cant save the document.

Even when I download the form and enter the values and try to save it ...it takes it successfully but when I close the doc and reopen it the filled values disapper.

Can any one help me out in resolving this issue plz.
