offline HTML as mail attachment

Could you please tell hoe to use HTML interactive form in GP.

the scenarios is:

User will initiate the process by clickiking of buttin in WD which has email address, GP sould send the the same email with HTML form as attachement.

After the form has filled and on click of sumbit, this sould send the data beck to iniatior. Could you please tell me how to configure HTML form attachment as callable object.
