Activation problem in PDF-forms

Hi to everyone!

I'm trying to make a simple PDF-form: to print data from SBOOK training table. I use BC480 course. I've already done Interface, Content and Layout, but I can't activate form! No errors were found during Check, but after activation I see message "Action canceled by user" and form remains inactive. I don't cancel any action. I even can't activate an empty form.

What could it be? Maybe somebody had such a problem?
