Purchase Order workflow sub tasks gets completed without any Outcome (Approve/Reject) in Log

Dear All,

I have an issue in Purchase Order workflow.When approve clicks on Approve/Reject in decision step ,there is no outcome in workflow log but it is showing that Decision step is completed and further steps are not executing. This issue is happening sometimes not for all PO's. Please find attached screen shot.

First check workflow Technical log , what message shows there. Share the screenshots.

In technical Log it is showing as step completed. and i have checked completed steps by that user in SWI5, it is showing as user has completed step. Please check attached screenshot.
