How to bring custom fields using Follow Up Button in CRM

Hi all,

I have a requirement to bring some custom fields along with its values (from AET Table) when clicking follow up button from Both BT Opportunity and Lead.

So, after i choose Follow up button in Opportunity, i have to choose some custom fields, once i choose those field values will goes to Appointment and save...

I can able to create Appointment by using follow up button in both Opportunity and Lead but not able to provide some custom fields to choose and finally custom fields will be saved to Appointment...
