What is the t-code flow or flow of creation of change notification from creating it till creating ecr from change notification.

Goto QM02 or QM03
fallow the menu path
extras -->Notification documents >Document flow> list or graphs
you can use QM19 also to see notification details in multi level

Are you looking for for some thing else than the answers given by me or another friend?
I am not sure about your answers since in sap help there is no mention about the QM part since anyone in organization can raise change notification that may get converted to approved change notification and it will then become as basis for ECR.
You can check with QM11 also.
