Table Maintenance Generator & Attaching to Transaction Code

I have a Custom Table and to be created with Table Maintenance Screen for user to Insert values into the Table. (1) Can you please provide me suggestion to create the Table Maintenance, (2) How to Attach a Transaction Code to the Table Maintenance? What kind of Transaction (Transaction Code Option) is this?
