Vendor Text and Internal Note in Shopping Cart

Hi

We are implementing SRM50. While creating a Shopping cart, after clicking on documents and attachments tab in item details; the text box for entering vendor text/internal note appears on the top left corner of the screen rather than at its proper place.

Any idea how to overcome this issue?

Unfortunately, the 3 suggested notes didn't solve the problem. Our support package level is 09 of SRM5.0
