How to add the extra fields

Hi friends this is Shalini Shah

iam having some problem i.e how to add the field to a table.

those fields are like TRUCK NUMBER

tranporter,

total WT/ Volume,

n. of cases/ boxes

so these are the fields.these fields i have to table .

please tell me how to add those fields
