how to create custom form in sap business by design?

i created some extention fields in personal admin___<>EMployement---->employee

now when i am enhancing fields to Custom form level it's not showing MY CUSTOM FORM TEMPLATE IT'S SHOWING EMPLOYEE PROFILE .could please tell me how to send THAT FIELDS TO CUSTOM FORM
