Udf mapping for multiple fields in csv file

Hi,

I need help in udf mapping

we are getting csv file in this format. Here the date fields are not fixed, each and every file we get multiple date fields like this 2017-01-22,2017-01-23,2017-01-24,2017-01-25,2017-01-26,,,,,..... Accordingly, the data for each and every date come in a row wise i.e. whether the employee is Attended/Sick Leave.

Require a UDF program in such a way that Mapped ID,NIN,Name,Duration will be one to one mapping and date fields for each employee should be posted in the database table as below output format.

Could anyone help me in this requirement
