how to attach script and standard text?

how to attach script and standard text for transporing from one client to another?

i have attached stanadard text in in script but im not sure how to transport these together to another client?

can anybody provide me an example program on adding a field to anyform ,i know we can use subroutines but im not sure how exactly we do!
