Why user is receiving two identical Test Control Effectiveness Plans if only one was created?

Hi!

I have some problems with the effectiveness workflow in GRC PC:

When I create a Test Control Effectivenes plan, the user who has the Plan assigned recives two identical plans in their Work Inbox .

Do you know the report/method in which the plans are created in order to debugging?

Is the workflow triggered twice for the same control? Which release and support pack you are on
