Questionnaire not showing in Lead in CRM

Hi All

I am not able to add Questionnaire in lead. I have done all necessary settings what need to done in Settings for Leads option. But when I open the transaction type Lead and go to customizing header it does not show any option to add the questionnaire and when I go back then the option shows block. Screenshot attached of customizing header, Block questionnaire and Determination for questionnaire of Lead .

Kindly Help.

Hi Surajit,

have you checked the "Active" - Flag within the "Define Determination for Questionnaires" Customizing?
