How to add certain pages or forms?

Suppose in ESS -> Appraisals

We have

My Appraisals

Under this we want to give a link which will display a form (say) 180 degree feed back form.

How can I do that?

The form may be

A)a pdf which is just printable. Do I have to make some extra settings to make a pdf printable?

B)it can be a form which will take input from the data base like loan or advance salary form which will take input from payroll. Where do I write the code to fetch data into the form?

C) it can be a form which will take input and then input the data of the form in some table in the ECC.

Where do I keep these forms? In portal or in ECC?
