service entry sheet layout

I want to create a new layout of service entry sheet which shall include the following additional fields other than inthe standard SAP layout:

1. Account assignment category.

2. All fields of "Value" and "history" tab.

How shall I add these field s in the standard SAP service entry sheet layout?

Please detail in steps.
