How to create a user specific template with common info in BSP

Hi Experts,

I need to create the document for the Annuitant (Retriees) through they will inform their annuity department still they are surviving....so based upon that,Annuitant department run the annuity payslip for him.......

If they died they will go for final settlement...so this is the business requirement for this document...

In that document, It holds common information like company information in the top and respective employee information like EMP ID and his contact address in the down......

For ex: If Employee "X" is Login into portal .That document should contain common information like company name and employee's "X" information like his EMP ID and his contact address .Like wise the form should generate both common address and employee respective information.....both in Arabic and in English....

Is there any readymade BSP or Iview for this (or) if anybody have developed same kind of BSP or somehow related to his scenario, pls send the code to shakthigin@yahoo.co.in

How to do that.....its very urgent.......

Help will be higly apppreciated.......
