how to upload survey using workbench into visit?

how to upload survey using workbench into visit, so that once the visit uploaded in system the survey started visible there in survey tab.

I've tried the object "Visit Follow Up and Related Items" and tried to import visit there, but its erroneous every time, it seems i am missing somthing.

can you please give me a step wise guide for this.
