Media Handling-error in file uploading using add file button

i'm trying to upload a file using CREATE STREAM in the following manner.

i gave the header name and header value.

im trying to add file from my local system.

the file im trying to attach marked up.

but the file is not imported.

and finally I'm getting this error when I'm trying to execute it.

please help me with this.
