BOPF enhancement - dumping on check

We have enhanced the BO - EHHSS_INCIDENT_REPORT.

After activating it - not able to add determination to it - it does not show create determination option.

Also when i try to check and generate or delete the enhancement it dumps. See pic.

We did add a custom field in the append and it is visible in the BO.
Any reason why we are not able to Add determination, delete the enhancement or check and generate the enhancement ?
