Hana Cloud 1909 EHS Initial incident Record Adobe Form not calling Custom OData created

Hi Team,

We have copied the EHS - Incident Management - Initial Incident Record Form which is Adobe fragment based form into a custom form, adding a couple of fields and updated the Form name in the SPRO settings.
We have redefined the OData service and added entities as per the requirement.
This Service has been added to the Copied Form as the Gateway Service and also it's node is activated.

The issue is that the newly created OData service is not getting called.
However the data is getting populated in the Standard fields as expected and the two added fields are visible in the Form generated.
There is a logic in the Data Provider Class which is populating the data.
I have debugged and found that neither the Standard Service nor the Custom one is getting called in this case.
I am not sure what is the missing link.

If anyone has worked on a similar requirement where the custom fields need to be added to a PDF form whose backend is an OData service then please help us to find out what we might be missing.
