Document Status is still SENT after the completion of leave workflow

Hi Experts,

I copied a workflow template WS12300111 to custom , In that standard task TS12300097 I removed an agent (&Agent_0001.Agents&) which is coming from business object AAGENT .So , I written a logic to get AGENT from custom class & method and assigned to standard task TS12300097. When AGENT(Manager) approves the leave the workitem is executed successfully but still SENT status in PTARQ_HEADER table only with one record and for field VERSION_NO the field value is 1.Only one record is exists for particular Request ID. I am struggling to resolve issue from past 1 month .Please help to resolve the issue ASAP.
