We are attaching documents in the appointments under activities. Is it possible to email the attachments to certain email address when the appointment is changed to status complete?

It is not possible to attachments directly in standard without enhancements.
You can create a Workflow with Email action .
In the final step,
Select the flag Add Attachment if you want to enable adding custom attachments to the e-mails sent using this workflow rule. Maintain the required logic to collect documents and attach them to e-mail. You maintain this customizing in the enhancement option: ExitForGettingWorkflowAttachment using SAP Cloud Application Studio.

Thanks for the response. Enhancement using cloud application studio is something we have never done and have to look into. As an alternate solution, is it possible to add a hyperlink in the email that will take us straight to the specific appointment?
Also for the trigger actions for appointments, I do not see Completed status. All I see is created, on every save. How can we get the Completed status as an option?

You can only deeplink a specific appointment and on clicking system will take you to that specific appointment but it won't get you to the attachment tab. This mail can only be sent to Employees as only they will be having access to C4C.

Created On and Every save are the timings.
Go to the next step, you can add status as a condition for the trigger.
