Hi,
You will see the changes if you click on Header Changes. (Environment -> Header Changes)
If you check the entries in table CDPOS and table CDHDR and do a merge by "'Doc. Number'' you will notice that both tables have the same document number. Open transaction SE16N, at "Object Value" you enter the puchase order number and compare the two tables.

2. Also did you check with any other user (admin user with sapall ) ?

Make sure to select purchase order item(s) before calling a menu to display "Item changes".

Are you sure you have made any changes to the purchase order that should be documented with change documents?
Also to mix change documents with purchase order history? Those are two completely different functionalities.
