To send multiple Satisfaction Surveys via workflow

When a user has a customer Visit on product. User met 3 contacts and talked to all 3 about a product in a meeting during a customer visit. In system user creates a Visit with 3 Contacts added to Visit and sets the Customer Visit to completed. Satisfaction Survey is automatically emailed to all 3 Contacts in that Visit (as per the custom workflow). But when the Survey form is filled and submitted by one of the contacts later on other contacts cannot go and finish their Survey form.

So is there any way three separate Satisfaction Survey forms instances can be sent to 3 contacts. Please see screen shots below for details?

Workflow rule

• 1.Created a visit and added 3 contacts to the Visit that I met and talked about a product

• 

• 2.Now contact 2 (test potter2) went to email (Outlook email ) and finished the Survey form
