Component scrap in production order is more than defined in BOM

Dear Experts,

We have defined component scrap lets ay 0.05% in bom for each components but in process orders system is showing 100% component scrap or in other words more components scrap defined in bom after issuance. Moreover this problem is not with all orders and Final issue indicator is not set. Please see below process order view of a material where requirement qty is 4.728 and withdraw qty is 1.60 and so the component scrap is 100% and same is showing in CK13N costing, however in BOM we maintained only 0.05%

What could be the reason? And how we resolve this ? and for future orders system should not set component scrap 100% but the defined % in the bom should be the scrap.
