Multiple dates to display based on the database record

In my report I have a field called date,for that field I have a requirement as shown in the below image

1)in the above image if the database contains single date value then it has to display one value.

2)in the second image,if the database contains two values then it has to display like above.

can anyone tell me how to achieve this?
