DataHub: Getting Duplicate publication errors & cannonical item id changing

Hi

I am getting Getting Duplicate publication errors & cannonical item id are also changing.What could be the reason,what is the steps to fix this ?

Attached is the publicationResult and error we are getting. This issue happened after we migrated from 5.6 to 6.1.
