Connection Type blank SQL Database import

I have followed all the instructions here:

However the Connection Type in the Create New Connection dialogue is empty (last step).

How do I troubleshoot this?

In case if you had missed, I would strongly recommend you to do so, as these steps helped us last time, when we were facing similar challenges.
