IDM 7.2 Error PVO task not working

Getting issue when provisioning new user where risk analysis is not being sent to GRC. This was working and it stopped all of a sudden. Here are the issues from the system log:

mxi_create_PVO: neither MX_APPLICATION_ID for 60825 (refers to master priv constant for QHR)

mxi_create_PVO: neither MX_APPLICATION_ID for 64277 (refers to master priv constant for ERP Quality client)

mxi_create_PVO: MX_PRIV_GROUPING_RULE missing (or with value 0) or GROUPING DISABLED for repid 40. Starting every pending task
