Troubles configuring Surveys

We are trying to implement surveys in our C4C tenant. We have scoped surveys, assigned the corresponding work centers and made the facets available on all the necessary views. Main area we want to attached surveys are to both Opportunities and Tickets. From my research, we need "Checklist" surveys.

I can only create a "Satisfaction" type survey.

What might we have missed?
