HI EXPERTS,
i want to know table of fields "Reference" "NOTES" "SURVEY" how to relate each of them on the basis of "task type" for task create.
If some points are missing then please let me know.

which field do you mean by "reference" in a task ?
The database table for notes is STXH.
Survey is not relevant, as I never heard about survey in a task already.
You can also use report crm_order_read to read the details of a transaction.
