In the Process Chain screen shot you provided that line has Process Type /CPMB/SWITCH_TO_PLAN, not /CPMB/FAIL_SWITCH_TO_PLAN.
It looks like you have discrepancy between Process Chain and DMP script that calls it. This can happen during upgrade or if you ran ENVIRONMENTSHELL activation for DM Packages. Problem is that it keeps PC and DMP in sync for delivered DMP only. I can see that your DMP is custom. So, you can try running program UJS_ACTIVATE_CONTENT with "Activate BI Business Content" checked or try activating LO PC from RSA1.
If after that you still don't have /CPMB/FAIL_SWITCH_TO_PLAN in your PC then try deleting your DMP and creating it again as Andy suggested.
