Frequently ADD ON get corrupt

Dear Experts,

Greetings!!!

We are using ADD ON on production order screen to create auto child work order creation and issue for production entry of main production order.

In this ADD ON frequently we are getting below mentioned image error and our implementation team said ADD ON get corrupt and reason is same user tried to login from 2 different system and hence it is getting corrupt. So please guide whether it is genuine reason or error in coding of ADD ON.
