Getting an error while clicking on TERMS and CONDITIONS, raising a IT declaration request in ESS.

Dear Experts,

Hope all of you are doing good.

We have customized the TERMS and CONDITION text (using feature 40GAD and SE61, dailog text) in ESS for IT declaration.

While raising IT declaration request in ESS and clicking on TERMS and CONDITIONS, system gives an error FORM does not exist.

Please find the screen shot for your reference and assist me how to fix this.

It is quite urgent and hoping speedy response from experts.
