Maintain custom form for Ticket Summary

Hello All,

Request insights on maintaining a custom form for Ticket output summary. I uploaded a custom form in "Administrator->Form Template Maintenance" and published the same in Service Request. Now when i try to click on Preview /Add summary it does not show me an option of selecting the standard or custom form for printing Ticket summary.

Request expert insights for the same.
