DTW "misses" columns in simple upload

I have a user defined table, @OM_COMMISSIONJOB, which contains three columns:

Code - nvarchar(30) - not null

name - nvarchar(30) - not null

U_OM_DateAdded - Datetime

We have created a CSV file with about 1400 records that looks like this:

Code,Name,U_OM_DateAdded

Code,Name,U_OM_DateAdded

1176,1176,12/31/2014

2075,2075,12/31/2014

2100,2100,12/31/2014

2103,2103,12/31/2014

2104,2104,12/31/2014

2105,2105,12/31/2014

As you can see, name and code are always the same.

When I try to process this through DTW it shows errors for every row stating that Name cannot be NULL.

The error file looks like this (stopped after 10 errors – if not DTW crashes after 39 items):

Code,U_OM_DateAdded

1176,1176

2075,2075

2100,2100

2103,2103

2104,2104

2105,2105

2107,2107

2108,2108

2109,2109

This makes no sense given that the Name column seems to not exist.

I also saved the file as a Tab delimited file. In this case the import ran without error but left the date field NULL. In this case it seems to "lose" the date field and can proceed because that field allows null values.

Can anyone help?
