EHS Saftey App, Could not connect to the server

Hello,

  I'm hoping that someone can help guide me in the right direction.  After working through the Admin Guide (Administrator's Guide for SAP EHS Safety Issue Version 1.2 2013.08.23) I thought that getting access to the EHS Saftey App would work.  I however continue to receive the error "Could not connect to the server"

From reading other posts on in the discussions forums I see that I need to have access to this URL

http://<sap and gateway server>:8001/sap/opu/odata/EHSMM01/LWM_SAFETY_OBSERVAT/

which does return without errors, however the app is not able to connect to the app.  in my environment we are using the netweaver gateway locally on the same SAP server that it is connected to.  We are also NOT using Afari nor SMP, which according to the admin guide are both optional.

I've looked over the installation and post-installation instructions but I'm certain there must be additional steps required in order to make the app work, I also looked at this post by Omar but I'm not using SMP so how do I set the endpoint?

I'm not certain why the guide doesn't state this information as part of the the Post-Installation steps but I was finally able to connect to the EHS Saftey App after following the guide and getting my LWM_SAFTEY_OBSERVAT to return without errors

I was missing the URL Suffix

URL Suffix: /sap/opu/odata/EHSMM01/LWM_SAFETY_OBSERVAT/?sap-client=<CLIENT ID>

The outstanding issue now is that while the app does load (and theoretically connect). I cannot submit a safety issue successfully I end up with a screen stuck at: "Sending safety issue to server..."
