How to link the phrase sets to table strucutre in Injury/Illness log

Hi Experts,

Am unable to find the new creation phrase set Table/filled name.am searching in CGAC tables/fields also am unable to find..

How to link the phrase sets to table structure in Injury/Illness log.

Pls let me know the process ,i want generate report from all this new phrase sets/Phrase.
