HCM P & F - Health plans 0167 issue

Hi Experts,

we are facing an issue in process and forms in health plans

The form is loaded with IT 0167 of subtype DENT with below records.

then after click on Check and send records not appearing as like above.

Please suggest to overcome this issue.
