Form 16 issue, after applyin note 2020266.

Dear Guru's,

I had updated patches in the system for tax credit and applied note 2020266 as the tax credit was not coming in form16. after changes i am getting extra fields and i am not sure how to use these fields. i have attached screenshots of these extra fields in red boxes.kindly help me to excute it correctly.
