CX_SY_REF_IS_INITIAL error in Shoping Cart SRM Workflow

Hi Friends,

   An getting this error at the end of the my workflow.

Have created my schema , it has 5 Approval levels , last one is the Automatic Approval (TS40007988)  by System.

Level Details:

1. Project Level

2.Category Level

3.HR Approval

4.Finance Approval

5. Automatic/ System Approval.

Not sure why at last after all approvals Levels its getting the below error.
