Uploading Performance appraisals doc using LSMW

Hi,

I am new to using the LSMW tool. Can anyone help me with the upload? I have a document with PERNR, Review Period Begin Date, Review Period End Date, Competencies rating, goals rating and overall rating as columns in an excel doc. Please let me know the necessary steps to upload into SAP using LSMW. I do have an idea about LSMW but never uploaded any doc or data before.
