Adding a button to an application tool bar

Hello folks,

My requirement is to add a button to a standard application tool bar. For example VA02 tcode.

Do I have to do that in any user exit??

Can anyone throw some light into that??

When the user clicks that button..where should I include my code for processing that user click??
